Jim Gross: Prealbumin is the same thing as TTR (Transthyretin). A blood test that checks your level of Prealbumin can validate Vyndamax efficacy. The higher your level, the more stablized TTR that you have circulating in your blood supply.

Amvuttra does the opposite. With that silencer drug your Prealbumin level drops quite a bit.

Managing advanced cardiac amyloidosis while on Vyndamax requires regular monitoring to track how well the treatment is working and how your heart is holding up. Here's a general overview of the tests typically used:

Imaging & Heart Tests:
- Echocardiogram — checks heart wall thickness, pumping strength, and blood flow
- Electrocardiogram (ECG) — monitors your heart's electrical activity and rhythm
- Cardiac MRI — provides detailed images of heart structure and any changes over time Blood & Urine Tests (Cardiac Biomarkers):
- NT-proBNP or BNP — rises when the heart is under stress; a key marker for tracking cardiac amyloidosis progression
- Troponin T or Troponin I — indicates heart muscle stress or damage
- Free light chain levels — helps monitor amyloid protein activity
- Liver function (ALP) — checks if amyloidosis is affecting the liver
- Kidney function & urine protein — monitors for kidney involvement

These biomarkers are especially important because they form the basis of the Mayo Clinic staging system used to track AL amyloidosis progression over time. Given the complexity of your diagnosis, your care team — likely including a cardiologist and an amyloidosis specialist — will determine the exact frequency of these tests. It's really important to report any new or worsening symptoms at every visit, as this helps guide decisions about your monitoring plan.

Always discuss this checklist with your doctor to make sure it's tailored specifically to your situation.
