My Kappa: Lambda Ratio has been slowly increasing over the past year. Now at 1.82 (ref range 0.26-1.65). My Oncologist suggested maybe making a change. The listed risks and protocol sure doesn’t sound very inviting.

To be fair, I had little choice in the matter… Nevertheless, with proper care and monitoring, I wouldn’t be especially worried about Teclistamab. It’s essentially CAR-T in a box. Are there risks of severe CRS/ICANS? Sure, but manageable in the right environment. Sounds like you’ve have time to think it through, so that’s a plus. One other thing, I was youngish (52) and pretty healthy other than the heart failure (IIIa disease).
I had CAR-T (Cilta-cel) in 12/23. I was in the hospital for 15 days in an over-abundance of caution due to cardiac concerns. CRS was Grade 1 and lasted about a day. Easily managed with Tocilizumab. When remission ends, I’ll probably lobby for a transplant, but failing that, I’d have zero qualms about Teclistamab. Bear in mind, I’m in NYC, and can walk to Sloan Kettering on a good day…

Sorry to hear your Kappa: Lambda ratio has been slowly increasing. I can't comment as I take nothing other than Gabapentin for peripheral neuropathy. Keep us updated with your changes. Mike
